The study, published in Clinical, Cosmetic and Investigational Dermatology, examined 12 men treated between 2022 and 2024. Among them, three patients aged 30 to 40 exhibited a consistent progression: severe nape scarring followed one to two years later by scalp-wide inflammation and the development of deep, furrowed folds. Biopsy results revealed a shared immune-cell profile across all three conditions, marked by an abundance of CD4-positive T cells and CD117-positive mast cells.

Lead author Dr. Sanusi Umar emphasizes that because these conditions are typically diagnosed and managed in isolation, physicians often miss the underlying systemic pattern. The presence of PIILIF—a specific pattern of lymphocytic inflammation and fibrosis—was found even in seemingly healthy scalp tissue, suggesting a widespread, underlying process. The researchers propose that the appearance of scalp folds should serve as a diagnostic red flag, prompting clinicians to perform comprehensive examinations and multisite biopsies to catch the disease before it advances to permanent hair loss or severe remodeling.
