Lightbeam and ChartSpan Merge Monitoring Tech for Value-Based Care

Dallas-based Lightbeam Health Solutions and ChartSpan have launched a strategic partnership to integrate device-based and deviceless remote patient monitoring. The collaboration aims to dismantle fragmented workflows by embedding continuous patient data directly into population health analytics, allowing risk-bearing providers to intervene before costly hospital readmissions occur.

The joint initiative combines Lightbeam’s AI-enabled population health platform with ChartSpan’s enterprise infrastructure for clinical data. By standardizing the ingestion of device and deviceless patient information, the companies intend to move away from isolated monitoring programs that currently plague many health systems. This architecture allows organizations to scale their coverage across entire patient populations rather than treating monitoring as a standalone technical challenge.

"Healthcare organizations are integrating personal health data in every direction—cardiology building one way, population health another, value-based care a third," said Drew Schiller, Chief Development Officer at ChartSpan. "That is not a program problem; it is an architecture problem." The partnership seeks to solve this by providing a unified foundation for closing care gaps and addressing the ongoing nursing workforce shortage. Patrick Burton, Senior Vice President of Business Development at Lightbeam, noted that the integration provides the technical and management capabilities necessary to reduce avoidable emergency department visits and hospitalizations, ultimately driving better financial outcomes for risk-bearing entities.
