The organization secured its wins as a community care hub within an eight-state consortium led by the Oregon Wellness Network and as the designated lead for the Texas Hub Network. By advancing to Phase 2, known as the Implementation Accelerator, CATCH will refine its service delivery through July 2027, aiming to reduce hospital readmissions and increase the time high-risk patients spend in their own homes.
Local implementation involves a collaborative network including the Houston Health Department, Memorial Hermann, Harris Health, CommonSpirit Baylor St. Luke's, and UT Physicians. These partners provide core services such as care transitions, nutrition support, and fall prevention programs. A technological shift underpins this expansion: all consortium hubs are adopting the AthenaOne electronic medical record, marking a rare instance of unified data systems in community-based care. UTHealth Houston School of Public Health will lead an independent evaluation of the initiative's success in managing patient costs and improving clinical outcomes.
