SARCOA and Community Care Solutions Secure Dual Federal Health Awards

Dothan-based SARCOA and Community Care Solutions have secured rare double recognition in the federal Health at Home Challenge. The organization earned a win as a lead regional team and as a key member of a national consortium, aiming to expand community-based healthcare across Alabama and the Florida Panhandle.

The Administration for Community Living selected the group as one of ten Phase 1 winners for their work in managing care for older adults and individuals with disabilities covered by Medicare and Medicaid. Serving an 18-county footprint, the team integrates hospital transitions, nutrition services, and fall prevention programs into a cohesive network. By partnering with Southeast Health Accountable Care Organization and Statera Health, the initiative seeks to reduce hospital readmissions and lower care costs through tighter coordination between clinical providers and local social services.

Following this recognition, the network plans to scale its operations into 16 additional Alabama counties. To support this growth, the consortium is adopting a unified electronic medical record system, AthenaOne, integrated with artificial intelligence for population health analytics. According to Kimberly Falkner, Executive Director of SARCOA/Community Care Solutions, these digital tools will allow care teams to identify at-risk individuals earlier and provide real-time support. The teams now move into Phase 2 of the federal competition, which runs through July 2027, with the potential to advance to final scaling based on their local performance.
