Medicare launches GLP-1 Bridge program for weight management

Starting July 1, 2026, eligible Medicare Part D patients will gain access to weight management medications Foundayo and Zepbound for a flat monthly cost of $50, marking a significant shift in federal coverage policy for obesity treatments previously excluded from broad Medicare support.

The initiative, titled the Medicare GLP-1 Bridge program, addresses a gap in care for the estimated 20 million Medicare beneficiaries who meet clinical criteria for obesity treatment. By providing a pathway for Eli Lilly’s orforglipron—a daily oral pill—and tirzepatide—the widely prescribed injectable—federal authorities aim to move beyond the one-size-fits-all approach that has historically limited older adults' access to specialized care.

Coverage eligibility requires that patients meet specific clinical criteria, including a Body Mass Index of 35 or higher, or a BMI of 27 with weight-related comorbidities. The program is designed to operate through December 31, 2027. Patients must obtain a prior authorization and confirm their specific Medicare Part D plan participates in the coverage pathway. While the program broadens access, it excludes individuals already receiving GLP-1 therapies for type 2 diabetes or those with certain pre-existing conditions that may already be covered under standard plan provisions.
