Seventy-two percent of technology leaders report that their AI initiatives are failing to meet expectations, according to a new report by Collibra and The Harris Poll. The findings suggest that the primary culprit behind these stalled deployments is not the models themselves, but a crumbling, unaligned data foundation.

The research, titled The 2026 Hallucination Tax Report, reveals that while 90% of organizations are actively deploying autonomous AI agents, 76% struggle to move those projects from pilot programs into full production. This gap is fueled by a persistent need for manual intervention: 87% of decision-makers admit their teams spend hours re-verifying the accuracy of AI-generated context. Felix Van de Maele, CEO of Collibra, describes this phenomenon as a "hallucination tax," where the cost of human oversight and rework effectively erodes the business value of automation.

Large enterprises feel the strain most acutely. Among organizations with at least $100 million in revenue, 64% report that manual review processes drain significant resources, and 96% point to poor data quality as the root cause of project failures. In response, companies are shifting their internal structures to prioritize governance. Over half of surveyed firms have moved their AI reporting lines closer to their data management teams, while 84% have established clear executive accountability for flawed or harmful AI outputs. As regulatory scrutiny intensifies, 90% of leaders are now aggressively preparing for new compliance standards by investing in data lineage and robust documentation to ensure transparency.
